NEW PATIENT DISCLOSURE REQUIREMENTS Public Health Law Section 24, effective March 31, 2015, requires health care professionals to make certain disclosures to patients and prospective client. A healthcare professional, group practice of health care professionals, a diagnostic and treatment center or certain health centers on behalf of health care professionals rendering services at the group practice, diagnostic and treatment center or health center must disclose to patients or prospective patients in writing or through an internet website the health care plans in which the health care professional, is a participating provider and the hospitals with which the health care professional is affiliated prior to the provision of non-emergency services and verbally at the time an appointment is scheduled.

We have no ownership interests, or ownership positions, or financial interests, in any rehabilitation facilities, PT/OT entities, massage therapy, occupational therapy, recreational therapy, medical transportation, MRI/Imaging centers, clinics, acute or sub-acute facilities, group homes or assisted living centers. All ancillary service vendors are encouraged to price as competitively as possible to enable utilization opportunities. However, the adjusters and ultimate payers reserve the right to negotiate with all service providers. Ava Care assumes no responsibility for this established industry practice. Our clients always guide their course of care and are welcome to explore all treatment options practicable to the recommendations of their treating physicians.

Ava works with all local and regional hospitals but is not an affiliated member of any Health Care organization, Managed Care Organization, Hospital, Clinic, etc… Ava Care & Case Management is also willing to consider bodily Injury claims pending settlement upon receipt of necessary due diligence documentation.
